Before Your Visit
A little preparation can make your first appointment easier. After scheduling, you’ll receive a patient registration link so you can complete your information before arriving.
If you have previous imaging, medical records, or notes related to your foot or ankle concern, bring them with you or send them ahead when possible. You may also want to bring any orthotics, braces, or footwear that are directly related to the problem you’re experiencing.
Please bring:
- Photo ID
- Insurance card, if applicable
- Referral information, if required
- A current medication list
- Relevant medical or surgical history
- Previous X-rays, imaging, or records related to your foot or ankle concern
