Your time is valuable, which is why we put forth every effort to ensure your scheduled appointment is kept. We ask that you provide as much notice as possible if you are running late or you need to cancel—typically no later than 24-hour notice is preferred. By providing an advanced notice of being late or cancellation, we can fit in another patient who needs further care. If you do not show up to your appointment, you might be charged a fee.

At Put Your Feet First, we are a provider for Medicare, as well as most major insurance plans. Before making an appointment with Dr. Forman, our Scottsdale, AZ podiatrist, we encourage you to confirm your coverage by calling our dedicated front office staff. Co-payments are due at the time of your appointment, and you will be responsible for any unpaid balance that is not covered by insurance.

How Do I Pay for My Appointment?

We are more than happy to submit insurance forms for you to help you recover the most from your benefits. At your visit, and with proper paperwork, we will do everything we can to help you afford the treatment you need and want from Dr. Forman, our podiatrist in Scottsdale.

If you require major work, a complete payment plan will be designed to meet your individual needs and for an appropriate payment schedule. The forms of payment accepted at Put Your Feet First are check, cash, or any major credit card.
